This page allows you to request that we prepare an application for an IRS Employer Identification Number ('E.I. Number', also known as a Federal Tax ID Number.) for your business. In order for us to complete your request, we need all of the information below. We will then mail you the appropriate IRS Application Form so that you can complete the application process.

NOTE: We can only process this form if you complete ALL of the fields below. When we receive this online form, we will then mail you the IRS Application Form. Do not leave any of the fields below blank.
NOTE: The form below is to attain the E.I. Number ONLY. If you are forming a company through us, DO NOT USE THIS FORM. You will be able to request an E.I. Number as part of the company formation process. Click here to form your company online instead.
